Apply for a personal licence to sell alcohol

A personal licence allows individuals to supply alcohol where a premises licence or a temporary event notice is in force.

Lost or stolen personal licences

If your personal licence is lost, stolen, damaged or destroyed, you can request a copy of your licence from us by letter if we issued the original licence.

Your letter requesting a replacement needs to be accompanied by the £10.50 fee, and where appropriate, a full explanation or statement declaring why you no longer have your personal licence. Please provide as much detail as possible, for example, your full name, address, and licence number (if known). For our contact details, see Where to send completed forms or letters.

If you are not requesting any changes to your personal licence at the same time as requesting a duplicate, this will be sent to the address we have on file and we will not usually require any further information.

If you need to change details at the same time as requesting a replacement personal licence (if you have moved to a new address, for example), we will need you to complete a change of name or address form. For more details, see Change the name or address.
